Just go to a CTS shop - if you already have an expired visa in your passport they'll do you a 6 month multiple in about 4 hours. Pay less or have more time to wait and you can get a single visit visa done in a day.
Average by Intention.

US and Pakistan are not a problem, even though I thought they would.
I handed in my passport for a US visa on Tuesday and I got it back at 10AM today with a 10 year visa!!!
It's even easier for a Pakistani visa. Just go there, sit at the visa office all morning until the consul will see you. Answer a could of questions and get a 5 year visa on the spot.
China visa is very simple if you go to China Travel Services. Hand it in before 2PM (at the Central Branch) and get it back at 6PM the same day. I think they're only issuing 6 months visas as a maximum now.

Sorry I misread.
Last time I went (3 weeks ago) Americans and Pakistanis both got their visas at the border. They only had to pay more for it.
